As a child, your parent or guardian may have requested and been granted access to your records. If so, this access will be disabled when you turn 16 and we need your permission to speak to your parent/guardian about any of your health issues.

By completing this form, our Doctors and Nurses can provide you with confidential advice and support on issues such as contraception (emergency and regular), general health, mental illness, weight issues, acne, sexually transmitted infections, drugs, smoking and alcohol problems etc. Your mobile number if you have one is especially important.

You can be assured any consultation with us will remain confidential.

Smoking Status

Smoking Cessation clinics are held at the Surgery on a weekly basis with the Stop Smoking Co-ordinator - you may self-refer to this clinic, please book an appointment at Reception. Nicotine replacement and other medications are available on prescription to assist you in stopping smoking and are available once you are under the care of the stop smoking co-ordinator.
